When Director Montgomery mentioned the "original location," he meant the calf.
Because it doesn't spread, it doesn't affect other parts of her body, and because it doesn't get metabolized and expelled, that's why Director Montgomery was able to discover this toxin.
This is also why only the nerve tissue in the calf has necrosis, while other areas are normal.
