The reason Tsunade's Lightning Release Ninjutsu is a Forbidden Technique is because if the user doesn't handle it well, it can cause self-harm, just like Multiple Shadow Clone Jutsu is also a Forbidden Technique. Both are Ninjutsu that can harm the user if not used properly.
Also, I originally intended to name this Ninjutsu Extreme Lightning Stimulation, but Extreme Lightning could easily be misread as 'chicken ribs' (meaning something useless).
